Release Form

Fair Credit Reporting Act (FCRA) requires that all agencies obtain a release form before performing a background check on any individual.

1.) Download the Personal Information Release Form by clicking on the link to the right.

2.) The subject of the background check must complete and sign the release form.

3.) Fax or Email the form back to Employment Screening, Inc.
